A comprehensive All-on-X quote should include the 3D CBCT scan and consultation, the surgical procedure with IV sedation or general anesthesia, the implant components themselves, the temporary prosthetic worn during healing, all follow-up appointments through the healing phase, and the final permanent prosthetic.
Many low quotes exclude one or more of these. The sedation gets billed separately. The temporaries are extra. The final prosthetic is quoted as a different line item after you're already committed to the case. By the time it's done, the "affordable" quote has grown significantly.
The prosthetic material is the biggest variable in the final price. Acrylic is less expensive upfront and easier to repair if it chips. It costs more to fabricate but usually costs less over the full lifespan of the restoration.

Surgeon training and experience are also real variables. Board-certified oral and maxillofacial surgeons and dentists with advanced implant training have invested significantly in developing their skills. That's reflected in their fees and in their outcomes.
Most dental insurance plans don't cover full-arch implant procedures, classifying them as elective or cosmetic. Some plans offer partial coverage for specific components such as the extractions or bone grafting when it's medically necessary. It's worth checking your specific plan, but most patients are paying out of pocket.
Medical insurance occasionally applies when there's a documented functional impairment, but this is uncommon and requires proper documentation and billing codes.
